Instructions for tele-medicine appointments:

Telehealth visits: 

On the day of your confirmed appointment, call our office at 860-572-9566.

Tell us: Your name, the time of your appointment, and your physician.

Video appointments:

Log in is easy 

Select the link for your physician.

You will be asked to enter your name and be in a virtual "waiting room".

Please wait online until your doctor starts the appointment video.

General appointment policies

All office visits are by appointment only.  Please call 860-572-9566 to schedule an appointment.  

If you have an ACUTE concern, please let the staff know and we will make every effort to see you that day.

If you need to reschedule or cancel an appointment our policy is at least 24-hour notice.

It is our office policy to charge $50.00 for any less than 24-hour cancelation,  missed, or  no show appointment. 

New Patients

Your first visit at Natura Medica will generally take about 1 hour to complete. Prior to scheduling your first appointment, please fill out the appropriate Intake form, HIPAA and Financial policies forms. Once we have your completed forms we will contact you for scheduling.

 

On or Before your initial appointment we will need:

  • a copy of your picture I.D. 

  • a copy of your Insurance card (front and back) 

  • copies of labs and /or imaging (XR, CT scan, MRI) for the last 3 years  

  • At your first appointment: Bring a list of supplements (option: bring actual bottles)  and prescription medications 

Health Insurance 

We are participating providers* with:

  • Connecticare

  • United Health Care

  • United Health Care/Oxford

*Insurance coverage is confusing so we encourage you to call the number on the back of your insurance card to confirm that you have "naturopathic care on your policy"

Co-pays are required at time of visit. We accept cash, personal check, debit, VISA, MASTERCARD, AMEX, Discover and HSA. 

 

Any other health insurance, payment is expected at the time of visit.

       

 

If you have a Health Savings Account (HSA) all of the services provided by Natura Medica are considered qualified medical expenses.

 

Pharmacy Refills​

Following your treatment plan with consistency will give you the best results. 

Options for refilling pharmacy: 

1) Go to PHARMACY tab for online ordering form 

2)Email your order to pharmacy@naturamedicamystic.com

3) Call the office 860-572-9566

PLEASE INCLUDE: 

1) your name 

2) patients name 

3) NAME of pharmacy product 

4) Number of caps or ounces per product 

5) Quantity you wish to order ( #number of each)
